I don't know if it counts as horror but Perfect Blue. Maybe more thriller than horror.
While we could always argue over what is horror vs something with horror elements, I think these are some of my faves:
When I discovered dorohedoro manga, I only stopped reading because I fell asleep be8ng too tired. It's such a good manga. It even has its own soundtrack!

Honestly this is the first time I found the bandcamp link for it. I only ever saved a playlist on YouTube lol
Seems to be legit when compared to these lists.

I think it's time for a reread
You've probably already discovered it, but Q Hayashida has a newish manga series out called Dai Dark that scratches the same itch.
No, actually. Thanks for the recommendation.
Takopi's Original Sin
Yeah its hard to compete with Takopi
Second favorite for me would be Hell Girl or Boogiepop Phantom
Higurashi or Another
It's a toss-up between The Promised Neverland and The Summer Hikaru Died.
I like how they did the horror elements in Hikaru. Actually gave me goosebumps on some scenes.
Very rare for horror stuff to look that good in an anime.
Hm, Higurashi, maybe. I don't watch a lot of horror anime, weirdly enough. I love horror stuff otherwise.
I don't watch a lot of horror, but The Summer Hikaru Died wins easily for me.

Best to go in blind.
Kaya-chan Isn't Scary, from the current season.
But don't take me serious, it's pretty much the only one I watched in this genre. And it's very much balanced with cuteness. But it really creeps me out.
There's a Junji Ito anime series! I think it's a Netflix production.
Mieruko-chan was pretty good, funny and spooky. A little bit hornier than I was looking for, but not too ecchi.
Mushishi isn't outright horror, but some of the plotlines are so unnerving they live with me rent-free.
I haven't seen it, but I've heard Monster is supposed to be good
evangelion, i guess you can say the old one. the new one can be horror depending on how you see it(not the ending, but the entry into mysterious realm minus space)
Monster
