I'm such a grammar nazi that not placing punctuation where it is needed is truly at the top of the list of things that irk me.

However, adding one where one is not needed is surprisingly high on that list as well.

Hot take incoming. The right panel is grammatically correct (but wrong for the syntax of the meme) and removing the comma changes the meaning.

Think of the setance as if it was the same structure as, "you are the one, John." In this case, John is being addressed and we need the comma. In the meme, the patient is being addressed, and instead of being "the one", they are "my most patient"

Not a hot take at all; we just read the sentence differently, and that's the joy of writing!

You could go further and add an additional 'patient' to the mix and have the Doctor say "You are my most patient patient, patient!"
