You could go through the following route:
If you don't have the means to get new hardware, you can use hyper-v or virtualbox for free on Windows or virtualbox/virt-manager on Linux on existing hardware.
Simple project to start with: install pihole and configure it as your DNS adblocking filter at home
While doing things like these, keep notes what you do by day, using those notes, put together your own howto manuals. Do not be afraid to fail, sleep over it and try again, don't be afraid to ask specific questions here or on other forums, IRC.

NoMachine (remote control software, server/client) nomachine.com (free for personal use)
RDP (Windows Remote Desktop)from work PC to personal PC or the other way around

That or get another set of peripherals?

Does your monitor have a USB hub?
Could use that and then swap the USB Hub cable between PCs, connect your Personal PC via Display Port and your home PC via HDMI, then switch using the monitor's menu button?

I'm sorry to hear you're using Discord, for me it fits right in with the likes of Reddit, Twitter and Facebook. Why not use something independant and federated like Matrix or XMPP?

My own house, internet line with ISP that is cool with selfhosting, Proxmox/TrueNAS, Opnsense/OpenWRT for network equipment, server hardware is asrock server board in one server, hp microserver gen8 for storage, https://zaggy.nl
If I didn't have the first 2 I would probably use a VPS.
