Hint: it's mostly toxic masculinity about a man letting (possibly another man) touch his balls or butt. Also your man boobs can get cancer whether you've got em or not (technically all of you got em whether obesity has offered them some extra adipose or not, hence the cells available to possibly cause cancer).
Perhaps it's the fear of knowing, and thinking if they are sick, it makes them less useful to their family and society. Plus got so much to get done IRL that you just push it back
This seems AI generated. The lower right of the billboard (which also usually aren't at fencepost level in the US), appears to have a backwards R or cyrillic characters in the stylized logos corner.
The department of Health and Human Services also doesn't use marketing like this in any of their (archived) public information campaigns, and like the other commenter said, the domain/site is bogus.
Also, why would a fence style gate (with what appears to be a nameplate on the bottom, Lamar) be used to mount a billboard in the first place?

This isn’t AI it’s been around for years and years
Can confirm, this was saved on my camera roll in 2015.
Quite stubborn

Yeah but that's mostly everybody these days.
This thread is making me adipose all over myself.

Not ai, this has been around since the early 2000s
Pretty sure its not Photoshop either, its grafitti on a real billboard.
And that's not a gate, that's a billboard. Full stop. Those are very standard along highways in farmers fields.
It existed prior to Aug 13,2014 and is probably photoshop.
ahrq.gov is a real site, what address is bogus?
seems like a bogus website
