You know you're in trouble when they don't even bother with vicious mockery, and just laugh. The damage bypasses your character and hits the player directly.
This would unironically be awesome at the end of a cosmic horror themed campaign. Some elder god the PCs have antagonized turns to the players and addresses them directly - not their characters, but the players - in a fourth-wall-breaking moment, acknowledging their existence and directly threatening them. Then the DM breaks out health and sanity bars for everyone at the table, and starts ticking off damage on them.
Exactly, that's how you know you're truly at the endgame boss! Instead of attacking your characters, someone challenges you directly... Anyway, roll for initiative.
BBEG: (Turns to his minions on the next turn) Now... CLAP.
Henchmen: (happily applauding)
Hapless nation of suckers: (sees their ancestral enemies being decimated by horrors beyond comprehension) (Frantically claps to appease this god-tier sociopath)
