There is an infamous movie about this called Ai no Corrida/In The Realm of the Senses mostly known for the unsimulated sex and yes, it does have the castration scene. But as a film, it's surprisingly good and has a depressing story.
While visiting a friend from Mexico City back in the mid-90s, I saw this film - completely uncensored - at the UNAM main campus cinema, called Ciudad Universitaria.
